Will your health information get swept away when disaster strikes?

Personal Health Record Content

 

Whether you are organizing your records or deciding to use an Internet electronic personal health record (ePHR), you should be sure that you have a complete and up-to-date record available prior to the hurricane season or all year.  At a minimum, your PHR should include:

  • Basic information about each family member (i.e. name, birth date, address, phone number, email address, social security number, etc.)
  • Physiological information (i.e. height, weight, hair and eye color, ethnicity, blood type, scares, birth marks, tattoos, etc.)
  • Names, addresses and phone numbers of people to contact in an emergency
  • Names, addresses and phone numbers of your health care providers
  • Health insurance information
  • Health risk factors and medical conditions
  • Medications and dosages
  • Allergies and drug sensitivities
  • Immunizations and their dates
  • Clinical tests and their results (i.e. lab tests, MRIs, xRays, etc.)
  • History of hospitalizations, surgeries and other medical procedures
  • Pregnancy history
