Creating a Personal Health Record can take time and persistence, but it is well worth the effort. There are good resources online to help you, such as the Step-by-Step Guide to Creating a PHR from the American Health Information Management Association’s MyPHR website (http://www.myphr.com/your_record/guide.asp). You can also take advantage of online resources to save your health care information either on paper or online. The following checklist is designed to help you keep track of important health-related information for your Personal Health Record. You can use this list to collect health records for yourself and your family. Florida law allows you to obtain copies of all of your medical records from every physician who treats you. By using this PHR checklist, you can ensure that you have a complete record of your and your family’s health history.
