Will your health information get swept away when disaster strikes?

Health Plan Personal Health Records

 

Some health plans in Florida that offer electronic health records (ePHRs) are shown below.  If you are a member, you might wish to take advantage of these services.  Be sure you understand how you would obtain access during a hurricane emergency and be prepared. 

  • Aetna:  Members may login to their personal health record by visiting http://www.aetna.com/ ActiveHealth’s Patented CareEngine® System, Aetna’s Personal Health Record, provides personalized alerts to members and physicians about opportunities to improve care, interactive functions and an online Health History Report.  Click on http://media1.aetna.com/wmedia/aet/web/AetnaPHR.wvx to view an introduction video.
  • Blue Cross Blue Shield of Florida:   Patients of physicians using e-Medicine, powered by RelayHealth®, can update their medical history via an online questionnaire, in advance of their office visit. The information is automatically stored in the patient’s RelayHealth Personal Health Record so patients and their physician can easily view and print it for their records. See: www.relayhealth.com.
  • Capital Health Plan:    Members can visit http://www.capitalhealth.com/ to sign-up or login to CHPconnect.  CHPConnect is an online tool to personalize and manage health care. With a CHPConnect account, members can: create a personal health history, keep up with your child's immunization shots, and track doctors' visits and tests, referrals and diagnoses, and other health services.
  • CIGNA:  Members can manage their health by logging into my.cigna.com.  At my.cigna.com members can store their personal health information, find a physician, compare hospitals, costs for procedures, dental treatments and more.   To view a myCIGNA video presentation click on https://my.cigna.com/portal/portal/guest/tour/Members_standalone.html
  • Humana:    The insured may go to www.humana.com and click on Members and then register for “MyHumana.”   The MyHumana program offers a “My health record” section where patient’s can enter their personal information.  Patient’s can review claims information, prescription information and history, plan details, and order ID cards.  
  • United HealthCare:   myuhc is available at https://www.myuhc.com/.  Members can view coverage information, track immunizations and allergies on your PHR.   Locate physicians and hospitals and compare quality and costs of procedures.  See personal and family claims history.
